DAYS/HOURS: Monday to Friday -  9:00AM-5:00PM24 Hour Emergency Service    The UCC health home care coordination program was set up by the Department of Health to offer our clients more services with their health care needs so that they can stay healthy.  The network includes more than 70 healthcare and social services providers, doctors, therapists, substance abuse counselors, hospitals, housing programs and other service providers, who work together to help our clients stay healthy. As a member, clients are assigned a care coordinator who will help them learn how to manage  and make decisions about their health, assist them in choosing services that will help them get healthy and stay well as well as to assist in connecting them with programs in their community. Creating behavioral health goals for the client. Documenting progress notes and assessments monthly. Developing a treatment plan with other care team members. Assessing and screening clients.
